Description: The Bloodthirster is a massive, bipedal creature standing over ten feet tall. Its body is covered in thick, scaly armor, and its long, muscular arms end in razor-sharp claws. Its head resembles that of a bull, complete with massive horns, and its eyes glow with an unholy red light. Its breath reeks of blood and death.

Nature: Bloodthirsters are primal predators that roam the darkest, blood-soaked corners of the world. They are driven by an insatiable hunger for flesh and blood, and will stop at nothing to satisfy their cravings. They are immune to most forms of magic, and their thick hide makes them nearly impervious to physical attacks.

Stats: The Bloodthirster is a CR 12 creature with the following stats:

Shapechanger: The bloodthirst can use its action to polymorph into a humanoid or back into its true form, which is a demon-like creature with a long, sharp-toothed maw.

Blood Frenzy: The bloodthirst has advantage on melee attack rolls against any creature that doesn’t have all its hit points.

Actions:

Multiattack:The bloodthirst makes two attacks: one with its bite and one with its claws.
Bite:Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 4) piercing damage. If the target is a creature, the bloodthirst can grapple it (escape DC 14).
Claws:Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 7 (1d6 + 4) slashing damage.
Blood Drain (1/day):Melee Weapon Attack: +6 to hit, reach 5 ft., one creature grappled by the bloodthirst. Hit: 10 (2d6 + 3) piercing damage, and the bloodthirst attaches itself to the target. While attached, the bloodthirst doesn’t attack. Instead, at the start of each of the bloodthirst’s turns, the target loses 10 (2d6 + 3) hit points due to blood loss. The bloodthirst can detach itself by spending 5 feet of its movement. It does so also if the target is reduced to 0 hit points or dies. A creature, including the target, can use its action to detach the bloodthirst.

Tactics: Bloodthirsts are relentless hunters that attack any creature they deem vulnerable. They use their blood frenzy ability to focus their attacks on weakened foes, and their blood drain ability to drain the life force of their prey. Bloodthirsts prefer to grapple their victims before using their blood drain ability, so they often try to initiate combat with a surprise attack.
